The welcome box looks kind of weak, seems like it’s just some small samples. Amazon also offers a free baby box, that box is apparently better and also comes with a 15% off code.
This promo just popped back in my mind. Did anyone get the box? Nope, have yet to get mine.

Doesn’t work if you go directly to the form – you need to go to Walmart’s baby registry page, find the Baby Welcome Box under Perks & guarantees, then click from there so the proper HTTP REFERER is set and the form will load.
