The following is a sample request for a credit freeze. Please make sure you fill in all of the information in [] and include the following attachments:
- Payment (e.g personal check, only if required – this table shows you what fees are required by each state)
- Proof of current address (e.g phone or water bill)
- Proof that you’re a victim of identity theft if needed under state law for the service to be free (e.g police report)
You should make a copy of this letter, then send the letter by certified mail and get a return receipt. Once the return receipt has arrived, attach it to your copy of this letter and keep it for your records. You’ll need to send a copy to each of the three credit bureaus, their addresses can be found at the end of this sample letter.
